Coming to Camp Health Form PDF

Please complete pages 1-3, being certain to fill in the form completely and to include a copy of your health insurance card. A licensed health professional must complete page 4; the actual exam can occur anytime after August 2007 (in the 12 months prior to camp).

Transportation Form PDF
Complete only if you need us to provide transportation to or from the airport.

French Broad River Assumption of Risk PDF (NOC Whitewater Rafting*)

Campers who are 13 years old and over must complete this form.

Sky Valley Farm Release of Liability PDF (Horseback Riding*)

Campers who are 15 years old and over must complete this form. (*Off-campus programs/providers)

Medication Packaging Procedure and Instructions PDF

A full page of detailed instructions. Applies only for daily prescriptions and daily over the counter medications. Please do not bring non-prescription medications unless they are taken daily. (We stock most over the counter medications already.) Please arrive at camp with individually packaged and labeled unit/dose bags for each date and time.
